Process to Obtain Family Card:

1. Visit our authorized information centre: Come to our centre and get guidance.

4. Get hard copy family card: Receive your physical Family Card.
5. Verify details on website: Check your details on our website to ensure accuracy.
6. Report mistakes via WhatsApp or centre: If there are errors, contact us via WhatsApp or visit the centre.
7. Card activation: Normally, the card is activated within 15 days , but we’ll try to help in emergencies .

फॉर्म भरें और जानकारी दें: फॉर्म भरें और उसके साथ मांगे गए जरूरी दस्तावेज (Documents) जमा करें।

ऑनलाइन फीस जमा करें: कार्ड बनवाने का प्रोसेसिंग शुल्क (Processing Fee) ऑनलाइन जमा करें।

फैमिली कार्ड प्राप्त करें: अपना असली ‘फैमिली कार्ड’ (Hard Copy) प्राप्त करें।

वेबसाइट पर जानकारी चेक करें: हमारी वेबसाइट पर जाकर अपनी डिटेल्स चेक करें ताकि सुनिश्चित हो सके कि सब कुछ सही है।

गलती होने पर हमें बताएं: अगर कार्ड में कोई गलती हो, तो व्हाट्सएप के जरिए या सेंटर पर आकर हमें सूचित करें।

कार्ड का चालू होना (Activation): कार्ड आमतौर पर 15 दिनों के अंदर चालू हो जाता है, लेकिन इमरजेंसी होने पर हम इसे जल्दी शुरू करवाने में आपकी पूरी मदद करेंगे।

फैमिली कार्ड कैसे काम करता है:

मेडिकल रिपोर्ट भेजें: कार्ड धारक अपनी रिपोर्ट्स और डॉक्टर की पर्चियां (Prescriptions) व्हाट्सएप या ईमेल के जरिए हमें भेज सकते हैं।

विशेषज्ञों की सलाह: हमारे एक्सपर्ट्स सही मार्गदर्शन के लिए व्हाट्सएप, कॉल या ईमेल के जरिए आपसे संपर्क करेंगे।

लोकल सपोर्ट: हमारा सूचना केंद्र आपको स्थानीय स्तर पर जरूरी जानकारी देने और तालमेल बिठाने में मदद करेगा।

अस्पताल में भर्ती और इलाज: अगर अस्पताल में भर्ती होने की जरूरत पड़ती है, तो हमारे पैनल अस्पतालों में मौजूद हमारी टीम (NCT Team) के साथ मिलकर आगे की प्रक्रिया पूरी करें।
